Outpatient Physical Therapist (Physical Therapy) Columbus, Ohio
The outpatient physical therapist provides comprehensive patient evaluations and develops, implements, and modifies individualized treatment plans in alignment with patient needs, goals, and physician direction. This role is responsible for delivering discipline-specific therapy services, maintaining accurate clinical documentation, supporting student learning as assigned, and upholding professional and safety-focused standards. Candidates are required to bring their license on the first day.

Job Details

  • Schedule: Days shift; 8 hours per day; minimum 40 hours per week.
  • Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ year of outpatient clinical experience; CPR certification; active/applicable license for the state where work will be performed.
  • Preferred Qualifications: Prior completion of an assignment with the organization is highly desired; prior completion of an assignment with a related organization is nice to have.
  • Responsibilities:
    • Completes comprehensive evaluations of assigned patients and uses clinical reasoning to develop treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
    • Completes medical record documentation using approved formats, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry, in accordance with state, federal, intermediary guidelines, and clinic policies and procedures.
    • Ensures care is provided under signed, valid physician orders and within required insurance approval, as indicated and necessary for clinical care.
    • Delegates and directs discipline-specific therapy services for assigned patients.
    • Participates in student programming as assigned, providing supervision, development of learning objectives, and completion of student evaluations.
    • Creates an individualized professional development plan and identifies professional growth opportunities through continuing education, self-study, and involvement in professional activities and organizations; applies learned information to support organizational objectives.
    • Adheres to the discipline-specific State Practice Act and other regulatory agencies; maintains professional licensure in good standing and completes state-mandated continuing education requirements.
    • Performs duties ethically in accordance with the discipline-specific professional code of conduct.
    • Demonstrates awareness of the organizations performance improvement methodology and follows action plans; provides feedback through staff meetings, focus groups, or direct leadership to support performance improvement outcomes.
