Job Details:
- Schedule: Days shift, 8 hours per day, a minimum of 40 hours per week.
- Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ year of outpatient clinical experience; CPR certification; active license in the state of Indiana; experience with patient evaluation, treatment planning, documentation, and compliance with state practice acts.
- Preferred Qualifications: Previous assignment with the staffing agency or organization is highly desired; professional development focused on outpatient therapy.
- Additional Information: Responsible for conducting comprehensive patient evaluations, developing individualized treatment goals, and maintaining timely, accurate medical records. Participates in student supervision and professional development activities. Adheres to organizational safety policies and maintains licensure in good standing. Demonstrates commitment to quality improvement initiatives.
