Job Summary:
Physical Therapist Outpatient (PT OP) position located in Ukiah, California. This role involves planning and executing segments of treatment programs aimed at restoring and enhancing patients' physical functions. The therapist will serve as a co-leader within an interdisciplinary team to deliver high-quality patient care, adhering to established clinical protocols and documentation standards.
Job Details:
- Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
- Required Qualifications: Doctor of Physical Therapy (DPT) degree; must have obtained license on or after January 2016, or foreign equivalent. A minimum of 3 years of outpatient Physical Therapy experience is preferred. Valid Physical Therapy licensure in the state of practice is required. CPR/BLS certification from an approved vendor is required.
- Preferred Qualifications: Pelvic floor therapy experience, PRPC or WCS certification or working towards certification, and 3 to 5 years of experience.
- Essential Functions: Conducts evaluations to ensure quality standards, utilizes appropriate clinical techniques and tools, establishes individualized treatment plans, and provides therapy interventions using validated protocols. Develops care and discharge plans, communicates plans to relevant parties, maintains comprehensive clinical documentation within deadlines, supervises assigned assistants, and performs other related duties as assigned.
- Additional Information: The position requires completion of evaluations, development of treatment plans, supervision of assistants, and thorough documentation. All clinical activities must meet department objectives and adhere to safety and compliance standards.
