Patient Care Coordinator Needed in Portland, OR
Pay Breakdown:
• Hourly: $81.25/hr Taxed
• Per Diem Eligibility: $1,687/wk
• $4,937/wk Gross Eligibility
Required Experience:
•OR License or License Waiver Eligible
•BLS
•1 year of case manager or PCC experience
•Weekends Sun-Sat w/holidays
•EPIC charting and experience with Medicaid/Medicare
•Will be expected to float to at least 1 neighboring location: Salem Hospital, Peace Health SW, OHSU, Legacy Salmon Creek.
•Previous KP experience preferred

•Schedule: 12am-8:30am
•GTD: 40hrs/wk
•13 weeks
•ASAP start
