Job Title: Pharmacist Profession: Pharmacy Specialty: Pharmacist Duration: 16-week assignment Shift: Day shift Hours per Shift: 8 hours Experience: Preferred one year's technical experience; inpatient clinical pharmacy experience preferred License: Pharmacist license in the state of practice required Certifications: All licensures and certifications needed to fulfill the duties required of a Pharmacist at the state level are required. Must-Have: Advanced Cardiac Life Support (ACLS) or equivalent, Basic Life Support (BLS) or equivalent, Pediatric Advanced Life Support (PALS) or equivalent Description: Verify and fill orders for medications. Monitor patient medication therapies. Provide drug information. Identify and troubleshoot potential medication safety hazards. Document all interactions for medical review. Supervise and direct support personnel. Process, enter, verify, and distribute medication orders within the institution in a timely manner and in compliance with policies and regulatory guidelines. Actively ensure that medication therapy is efficacious, safe, cost effective, and achieves the best therapeutic outcomes. Assure that medication prescribing is optimal with respect to indication, dose, route, frequency, and history of allergy, and does not duplicate or negatively interact with other drugs. Conduct and manage daily reports on medication utilization to identify and analyze areas in which medication improvement is needed. Perform quality assurance procedures in related work areas. Participate in pharmacy-related tasks and programs such as clinical rounds and medication management activities. Expedite changes needed to meet regulatory, accrediting, licensing, and government agency standards and laws. Contribute to department cost savings initiatives. Comply with drug formulary and maintain appropriate documentation. Collaborate with appropriate leadership to procure medications and related supplies based on the highest medication safety standards. Assist in making recommendations on process improvement or development of new pharmacy services. Perform other job-related duties as assigned.
