Job Title: Pharmacist Profession: Pharmacist Specialty: Pharmacist Duration: 16 weeks Shift: Day Shift Hours per Shift: 8 Experience: One year of technical experience preferred, inpatient clinical pharmacy experience preferred License: Pharmacist license in the state of practice required Certifications: Advanced Cardiac Life Support (ACLS) or equivalent required Basic Life Support (BLS) or equivalent required Pediatric Advanced Life Support (PALS) or equivalent required Must-Have: Graduate of school of pharmacy required Description: Verify and fill orders for medications. Monitor patient medication therapies. Provide drug information. Identify and troubleshoot potential medication safety hazards. Document all interactions for medical review. Supervise and direct support personnel. Process, enter, verify, and distribute medication orders in a timely manner and in compliance with policies and regulatory guidelines. Ensure that medication therapy is efficacious, safe, cost effective, and achieves the best therapeutic outcomes. Assure that medication prescribing is optimal with respect to indication, dose, route, frequency, and history of allergy, avoiding duplication or negative interactions with other drugs. Conduct and manage daily reports on medication utilization to identify and analyze areas needing improvement. Perform quality assurance procedures consistently in related work areas. Participate in pharmacy-related tasks and programs such as clinical rounds and medication management activities. Expedite changes needed to meet regulatory and licensing standards. Participate in department cost savings activities. Comply with drug formulary and appropriate documentation standards. Collaborate with leadership and procurement teams to ensure medication safety standards are met. Assist in making recommendations for process improvement or the development of new services. Perform other job-related duties as assigned.
