Job Title: Pharmacist Profession: Pharmacy Specialty: Pharmacy Duration: 8 Hours Shift: Variable Shift Hours per Shift: 10 Experience: One year’s technical experience preferred, inpatient clinical pharmacy experience preferred License: Pharmacist license in the state of practice required Certifications: All necessary licensures and certifications required to fulfill the duties of a Pharmacist Description: Verify and fill orders for medications. Monitor patient medication therapies. Provide drug information. Identify and troubleshoot potential medication safety hazards. Document all interactions for medical review. Supervise and direct support personnel. Processes, enters, verifies, and distributes medication orders in a timely manner. Ensures compliance with department and hospital policies, as well as regulatory guidelines. Actively ensures that medication therapy is efficacious, safe, cost-effective, and achieves the best therapeutic outcomes. Assures that medication prescribing is optimal concerning indication, dose, route, frequency, history of allergy, and avoids drug duplication or negative interactions. Conducts and manages daily reports on medication utilization based on daily rounds. Identifies, catalogs, and subsequently analyzes areas for medication improvement. Consistently performs quality assurance procedures in related work areas. Participates in pharmacy-related tasks and programs such as clinical rounds and medication management activities. Expedites changes to meet regulatory, accrediting, licensing, and government agency standards and laws. Participates in departmental cost savings initiatives. Complies with drug formulary and appropriate documentation. Collaborates with leadership and buyers to procure medications and related supplies. Assists in making recommendations on process improvement or development of new pharmacy services. Performs other job-related duties as assigned.
