Responsibilities
- Review, verify, and accurately dispense medications in accordance with physician orders and clinical guidelines
- Evaluate medication regimens for therapeutic appropriateness, drug interactions, allergies, contraindications, and effectiveness
- Prepare and compound medications, including sterile preparations, when required
- Monitor patient medication therapies and recommend clinical interventions to optimize outcomes
- Provide medication counseling and education to patients, caregivers, nurses, and healthcare providers
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams to ensure safe and effective medication management
- Participate in medication reconciliation, antimicrobial stewardship, and pharmacy quality improvement initiatives
- Maintain compliance with California Board of Pharmacy regulations, Joint Commission standards, and facility policies
- Document pharmaceutical interventions and patient care activities accurately in the electronic medical record (EMR)
- Supervise and support pharmacy technicians while maintaining efficient pharmacy workflow
- Assist with medication inventory management, controlled substance monitoring, and pharmacy operations
- Promote patient safety through accurate verification, clinical review, and medication error prevention
Requirements
- Active California Pharmacist License
- Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy from an accredited program
- Minimum 2 years of recent hospital or acute care pharmacist experience
- Current BLS Certification preferred
- Experience with electronic medical records (EMR), computerized physician order entry (CPOE), and pharmacy information systems
- Knowledge of sterile compounding, IV admixture, and medication safety practices preferred
- Strong clinical knowledge of pharmacotherapy and evidence-based medication management
- Excellent analytical, communication, and problem-solving skills
- Ability to work independently while collaborating effectively with multidisciplinary healthcare teams
- Commitment to delivering safe, accurate, and patient-centered pharmaceutical care
