Responsibilities
  • Review, verify, and accurately dispense medications in accordance with physician orders and clinical guidelines
  • Evaluate medication regimens for appropriateness, drug interactions, allergies, contraindications, and therapeutic effectiveness
  • Prepare and compound medications, including sterile preparations when required
  • Monitor patient medication therapies and recommend clinical interventions to optimize outcomes
  • Provide medication counseling and education to patients, caregivers, nurses, and healthcare providers
  • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams to ensure safe and effective medication management
  • Participate in medication reconciliation, antimicrobial stewardship, and quality improvement initiatives
  • Ensure compliance with California Board of Pharmacy regulations, Joint Commission standards, and facility policies
  • Maintain accurate documentation using pharmacy information systems and electronic medical records (EMR)
  • Supervise and support pharmacy technicians while maintaining efficient pharmacy workflow
  • Assist with inventory management, medication storage, and controlled substance monitoring
  • Promote patient safety through accurate verification, clinical review, and medication error prevention
Requirements
  • Active California Pharmacist License
  • Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy from an accredited institution
  • Minimum 2 years of recent hospital or acute care pharmacist experience
  • Current BLS Certification preferred
  • Experience with electronic medical records (EMR), computerized physician order entry (CPOE), and pharmacy information systems
  • Knowledge of sterile compounding, IV admixture, and medication safety practices preferred
  • Strong clinical knowledge of pharmacotherapy and evidence-based medication management
  • Excellent analytical, communication, and critical thinking skills
  • Ability to work independently while collaborating effectively with multidisciplinary healthcare teams
  • Commitment to delivering safe, accurate, and patient-centered pharmaceutical care
