Travel Pharmacist - $3,374 per week in Lodi, CA

Job Title: Pharmacist Profession: Pharmacist Specialty: Pharmacist Duration: 13 weeks Shift: Variable Shift Hours per Shift: Variable Experience: Minimum of one year of technical experience preferred; inpatient clinical pharmacy experience preferred License: Pharmacist license in the state of practice required Certifications: - Advanced Cardiac Life Support (ACLS) or equivalent required - Basic Life Support (BLS) or equivalent required - Pediatric Advanced Life Support (PALS) or equivalent required Must-Have: - Graduate of school of pharmacy required - Doctor of Pharmacy (PharmD) degree preferred Description: Verify and fill orders for medications. Monitor patient medication therapies. Provide drug information. Identify and troubleshoot potential medication safety hazards. Document all interactions for medical review. Supervise and direct support personnel. Process, enter, verify, and distribute medication orders in a timely manner. Ensure compliance with department and hospital policies and regulatory guidelines. Actively ensure medication therapy is efficacious, safe, cost-effective, and achieves the best therapeutic outcomes. Assure medication prescribing is optimal with respect to indication, dose, route, frequency, and history of allergy. Conduct and manage daily reports on medication utilization based upon daily rounds. Identify, catalogue, and analyze areas in which medication improvement is needed. Consistently perform quality assurance procedures in related work areas. Participate in pharmacy-related tasks and programs such as clinical rounds and medication management activities. Expedite changes needed to meet regulatory, accrediting, licensing, and government agency standards. Participate in department cost savings initiatives. Comply with drug formulary and appropriate documentation. Collaborate to purchase, contract, and procure medications and related supplies. Assist in making recommendations on process improvement or development of new pharmacy services. Perform other job-related duties as assigned.
