Travel Pharmacist - $3,116 per week in Clearlake, CA

Job Title: Pharmacist Profession: Pharmacist Specialty: Pharmacy Duration: 13 Shift: Day Shift, 4 x 10s, 4 days a week, no set days and including weekends occasionally Hours per Shift: 6:30 am to 5:30 pm Experience: Experience in hospital pharmacy practice preferred; Graduate of an accredited college of pharmacy; Residency training preferred License: Current California Pharmacist License Certifications: N/A Must-Have: Able to interface with hospital staff and the public in a courteous and professional manner; Demonstrates emotional maturity, self-awareness, and consistently performs with integrity and respect; Must have tact, good judgment, and self-motivation, be detail-oriented, and respect and protect the confidentiality of sensitive material Description: Performs and is responsible for the day-to-day professional activities of the Pharmacy Department including duties involving compounding, admixing sterile products, drug consultations, dispensing, providing drug information, and monitoring patient drug therapy. Job is represented by a union. Conducts and accurately documents in the medical record drug protocols as ordered by physicians, including but not limited to TPN, Pharmacokinetic, and Heparin. Contacts and intervenes with physicians and/or nurses regarding questions, errors, or irregularities on orders. Maintains and meets expectations on time for all competencies, licenses, certifications, and education requirements as outlined by local administration and regulatory agencies. Maintains computerized patient record system to monitor medication therapy for safety and efficacy, including but not limited to medications, medication errors, and nursing unit inspections. Maintains daily records of controlled drugs as required by law. Monitors and intervenes on all drug regimens as indicated by the quality assurance plan, including but not limited to drug-drug interactions, drug allergies, toxic/therapeutic drug levels, and appropriateness indications. Provides clinical drug information to meet the needs of the physicians and nursing staff through direct contact and/or committee participation. Reviews inventory levels to ensure that all outdated drugs or drugs that have lost their potency are removed. Supervises support personnel in routine performance of their duties, including but not limited to IV admixtures, medication dispensation, and medication compounding. Willingly performs other duties and innovations as assigned. This department controls, mixes, compounds, and distributes drugs, and provides pharmaceutical consultation in support of inpatients, as well as to ambulatory areas including ambulatory/outpatient clinics, infusion therapy, infusion oncology, ambulatory dialysis, ambulatory surgery, and operating rooms. Prescription-based retail entities are excluded.
