License: Current California Registered Pharmacist license
Certifications: Required as applicable based on specialty area
Must-Have: Completion of residency program in specialty areas such as Emergency Medicine, Infectious Disease, Critical Care, or Ambulatory Care is preferred as applicable.
Description:
The Pharmacist will review prescriptions for appropriateness.
The pharmacist will profile, validate, and dispense medications, supplies, and equipment orders accurately and efficiently.
Federal and state law, as well as approved hospital policies and procedures regarding medication use, must be followed.
Consultation with other healthcare providers and information resources will be done to optimize therapy and maximize safety for medication administration.
The pharmacist will answer all inquiries from clinical staff regarding medication issues.
They will guide, direct, and assist in training pharmacy technicians, new pharmacist personnel, interns, and/or residents.
Responsibility for the quantity and quality of their work will be assumed.
Medication therapies and clinical charts will be monitored and assessed.
Interventions in patient care will be implemented when indicated to optimize pharmacotherapy.
The pharmacist will participate in interprofessional patient care rounds.
Annual goals will be set and reviewed with management.
In the outpatient setting, the pharmacist will also provide consultation and actively participate in collaborative management of referred patients.
Ongoing participation in committee meetings related to pharmacy practice will be expected.
The pharmacist will assist in the development and implementation of policy and procedures to ensure safe medication use.
Clinical interventions will be documented regularly, and the effectiveness of the services provided will be measured.
The pharmacist will serve as a preceptor for pharmacy residents and interns, as assigned.
Continuous improvement in pharmacy practices and patient care will be sought.
