Duration: Long-Term Contract
Schedule: Full-Time, 40 Hours/Week, 8–12 Hour Shifts between 05:30 AM – 9:00 PM
Key Responsibilities:
- Provide clinical pharmacy services and medication management for cardiology patients.
- Review medication regimens for dosing, interactions, allergies, monitoring, and safety.
- Manage anticoagulation therapy (warfarin, DOACs, heparin) according to protocols.
- Provide medication therapy management, patient education, and adherence counseling.
- Document clinical recommendations and interventions in the medical record.
- Collaborate with physicians and multidisciplinary teams to optimize drug therapy.
- Educate staff and support pharmacy clinical programs.
