Maintain adequate drug supply, oversee procurement of medications, manage controlled substance accountability, and ensure compliance with all applicable Federal and State pharmacy laws, rules, and regulations
Prepare, sterilize, label, and dispense parenteral and non-parenteral medications; fill and verify drug orders including stat orders; and ensure proper labeling and expiration dating of all sterile products
Conduct monthly inspections of patient care units and pharmacy areas; monitor refrigerator temperatures; and oversee removal and documentation of outdated or recalled medications
Participate in patient drug therapy monitoring including review for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose; detect and report adverse drug reactions
Perform medication reconciliation for discharge medication lists, including gathering prior-to-admission home medication lists from patients, families, or pharmacies, and provide discharge counseling
Participate in Pharmacy and Therapeutics Committee meetings, quality assurance programs, medication use review activities, and new employee training and education programs
Required Qualifications
Active Pharmacist license valid in the state of South Carolina
Working knowledge of all areas of pharmaceutical services including drug dispensing, documentation, and regulatory compliance
Knowledge of controlled substance regulations and recordkeeping requirements
Strong communication skills for patient/family interaction, medication education, and interdisciplinary collaboration
Preferred Experience / Skills
Experience with EPIC electronic health records system
Clinical expertise in Antimicrobial Stewardship, including interpretation of culture results and knowledge of UTI, pneumonia, and SSTI treatment recommendations
Experience with Pharmacokinetic Dosing Management and Monitoring
Experience with Anticoagulation Dosing Management and Monitoring
