Contract | Days | Full-Time | Onsite | Shifts: 6:30 AM – 7:00 PM (12-hour) or 8:00 AM – 4:00 PM | Rotating weekends and holidays | No on-call requirement

Key Responsibilities

• Review medication orders for drug therapy appropriateness, including evaluation of dose, dosage form, regimen, therapeutic duplication, and drug interactions
• Provide pharmacokinetic and anticoagulation dosing services
• Participate in clinical pharmacy services including medication reconciliation and discharge medication education
• Oversee pharmaceutical compounding in accordance with USP regulatory and compliance guidelines, with focus on USP 797 and USP 800
• Support clinical areas including ED, ICU, Med/Surg, Behavioral Health, and Surgical Center, with limited focus on infusion center operations
• Collaborate with pharmacy technicians and interdisciplinary care teams to deliver comprehensive pharmaceutical care

Required Qualifications

• Active Wisconsin Pharmacy License in good standing
• Recent acute hospital experience required
• Proficiency with EPIC electronic health records system
• Experience with Automated Dispensing Cabinets (ADCs)
• Familiarity with Omnicell medication dispensing systems

Preferred Experience / Skills

• Experience in a critical access hospital setting
• Background in clinical pharmacy services across multiple care areas including ICU and ED
• Knowledge of USP 797 and USP 800 compounding compliance standards
