Maintain adequate drug supply, oversee procurement of medications, manage controlled substance accountability, and ensure compliance with all applicable Federal and State pharmacy laws, rules, and regulations
Prepare, sterilize, label, and dispense parenteral and non-parenteral medications; fill and verify drug orders including stat orders; ensure proper labeling and expiration dating of all sterile products
Monitor patient drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose; detect and report suspected adverse drug reactions
Conduct monthly inspections of patient care units and pharmacy areas; ensure cleanliness and order of the pharmacy; check and record refrigerator temperatures
Participate in medication use review, quality assurance programs, Pharmacy and Therapeutics Committee meetings, and implementation of committee decisions
Provide discharge counseling and medication education to patients and families; gather prior-to-admission home medication lists; perform medication reconciliation for discharge medication lists
Required Qualifications
Active and unrestricted Pharmacist licensure in the state of West Virginia
Working knowledge of all areas of pharmaceutical services including drug preparation, dispensing, documentation, and regulatory compliance
Experience with EPIC electronic health records system
Strong clinical knowledge including antimicrobial stewardship, interpretation of culture results, and management of common infectious conditions (UTI, pneumonia, SSTI)
Proficiency in pharmacokinetic dosing management and monitoring
Proficiency in anticoagulation dosing management and monitoring
Preferred Experience / Skills
Experience with medication reconciliation processes in an inpatient or acute care setting
Demonstrated ability to provide effective patient and family discharge counseling and medication education
Familiarity with JCAHO accreditation standards and quality improvement initiatives
