Opportunity:

Pharmacist

Job Details:

• Follow all safety protocols including 10 Foot Circle of Safety guidelines• Work under general supervision of the Director providing comprehensive pharmaceutical services
• Must be licensed in both Kentucky and Ohio
• Minimum two years of recent hospital experience required
• BLS certification required if mandated by state
• EPIC experience required
• Maintain adequate drug supply and inventory at all times
• Order appropriate quantities of medications on a timely basis
• Complete annual inventory process and forward results to Business Office
• Prepare, sterilize, and label parenteral medications and solutions
• Fill and dispense all medications according to physician orders
• Maintain strict control and accountability for all dispensed drugs
• Conduct monthly inspections of patient care units and pharmacy areas
• Manage controlled substances and maintain required documentation
• Perform medication reconciliation for discharge medication lists
• Provide discharge counseling to patients and families
• Monitor patient drug therapy regimens for contraindications and interactions
• Participate in antimicrobial stewardship programs
• Manage anticoagulation dosing and monitoring
• Participate in Pharmacy and Therapeutics Committee meetings
• Maintain patient confidentiality and computer systems
