Travel Pharmacist – Pharmacy Acute Care – Variable (4x10)

Key Responsibilities

• Review patient records to determine the appropriateness of medication therapy and evaluate patient conditions to ensure all clinical issues are being addressed
• Develop effective medication plans that minimize the risk of adverse side effects
• Consult with prescribers, nurses, and patients regarding medications, dosing, administration, side effects, incompatibilities, formulary considerations, and current care path management
• Assess the results and outcomes of pharmaceutical treatments and collaborate with healthcare professionals to ensure optimal patient care
• Maintain accurate documentation of medication plans and patient progress
• Participate in the education of students and residents as necessary

Required Qualifications

• Active Registered Pharmacist license in the state of Oklahoma
• Minimum of 2+ years of pharmacist experience
• Proficiency with pharmacy automation and dispensing systems (e.g., McKesson Automation, Omnicell Systems, ScriptPro, Baxa Systems, POS)
• Experience working with EPIC EMR
• Ability to work a variable 4x10 schedule including night shifts

Preferred Experience / Skills

• Prior experience in an acute care hospital pharmacy setting
• Experience working with medical patient populations
• Familiarity with care path management and formulary oversight in a high-volume facility
