Inpatient Pharmacist

Key Responsibilities
  • Review all prescriptions for appropriateness of therapy, legality, completeness, contraindications, interactions, allergies, adverse drug reactions, and lab value abnormalities; follow up by screening patient charts
  • Provide clinical consultation and clarification, resolving concerns or questions with prescribers prior to dispensing; document all clinical activities and interventions accurately
  • Counsel patients clearly and accurately regarding their medications, appropriate outcomes, drug name, strength, usage, and expected changes; verify patient understanding
  • Perform chart reviews for prescription refills and medication therapy management; write recommendations for providers and provide drug information and staff education related to drug therapy
  • Direct and verify the daily activities of pharmacy support personnel and technicians; serve as a role model and educator for pharmacy students and technicians
  • Participate in clinical and administrative activities including quality control, quality improvement, committee work, and pharmacy-run clinics; ensure availability, preparation, and control of medications
Required Qualifications
  • Pharm.D. degree and licensure as a registered pharmacist; candidates licensed prior to 2003 with a Bachelor of Pharmacy degree will be considered with a minimum of 3 years of experience in hospital and/or ambulatory clinic pharmacy
  • Current, unrestricted license as a registered pharmacist in the State of Alaska
  • Current flu vaccination required
  • Certification in Basic Life Support (BLS) for Healthcare Providers
  • Proficiency in reviewing patient medical history, labs, and medication orders using in-depth knowledge of pharmaceutical sciences
  • Strong knowledge of state and federal laws governing the practice of pharmacy; ability to work independently, set priorities, and respond quickly to emergency requests
Preferred Experience / Skills
  • Completion of a nationally recognized (ASHP-accredited) pharmacy residency program
  • Experience in hospital and/or ambulatory clinic pharmacy settings
  • Ability to handle frequent interruptions and adapt to changes in workload and work schedule
