Key Responsibilities

• Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction, utilizing 60-minute evaluation and treatment sessions
• Complete medical record documentation in a clear, concise, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific state practice acts and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
• Actively engage in professional development and organizational performance improvement initiatives, contributing feedback through staff meetings, focus groups, or direct supervision channels

Required Qualifications

• Active Physical Therapist (PT) license in good standing in the state of Ohio
• Experience or specialized training in pelvic health physical therapy
• Ability to complete thorough clinical documentation in compliance with state, federal, and intermediary guidelines
• Demonstrated ability to work within a multi-site outpatient setting, including floating to an additional clinic location as needed

Preferred Experience / Skills

• Prior outpatient clinical experience with a focus on pelvic floor rehabilitation or women's health
• Experience supervising and mentoring physical therapy students
• Familiarity with functional outcome measures and quality indicator documentation
