Requirements:

Valid and unrestricted Florida Physical Therapist license
Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
Active BLS certification issued through American Heart Association (AHA)
Florida ACHA fingerprint background check completed prior to start date
Current immunizations including MMR, Varicella, Hepatitis B, TB testing, and seasonal influenza
Clean professional license with no current or prior disciplinary actions
Completion of specialty competency exam and Joint Commission Core I, II, and III modules
