Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, develop, and implement individualized patient treatment plans
• Document patient progress and maintain accurate clinical records in accordance with facility requirements
• Collaborate with the care team to ensure continuity and quality of patient care
• Adhere to all facility policies, compliance standards, and credentialing requirements

Required Qualifications

• Active Physical Therapist license required
• Experience in an outpatient physical therapy setting
• Ability to meet facility credentialing and documentation requirements prior to start
• Must obtain a permanent state license; compact license may be accepted initially but permanent licensure must be pursued immediately upon placement
• Candidates must provide proof of license application within 5 business days of booking if permanent license is pending

Preferred Experience / Skills

• Prior experience working in a high-volume outpatient therapy environment
• Familiarity with electronic medical record documentation systems
