Job Summary:
Physical Therapist Home Health (Home Health PT) position located in Seaside, OR and surrounding areas. This opportunity involves providing skilled physical therapy services to patients in a home health setting with a focus on mobility, recovery, and patient education. The position starts as soon as possible and requires adherence to specified scheduling and operational protocols.

Job Details:

• Schedule: 8-hour days, starting on 10/19/2026 and ending on 01/18/2027, with a minimum of 40 hours per week.
• Required Qualifications: Valid state license (compact license accepted), at least 1 year of skilled home health experience, BLS certification, and willingness to use EPIC and Rover charting systems.
• Preferred Qualifications: Home Health experience preferred.
• Patient Ratios: May visit up to 6 patients per day; standard productivity points apply.
• Schedule Details: The facility cannot accommodate 4x10s or 3x12s; the assigned shift is 8 hours daily.
• Additional Information:
  • Must have own transportation; occasional maximum radius of roughly 60 miles per day.
  • Must be able to work primarily within a 50-75 mile radius.
  • Candidates are expected to commit to working 3 of the 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Years Eve, New Years Day).
  • The facility is a home health setting utilizing EPIC and Rover electronic charting systems.
  • No float requirement explicitly indicated; travelers are generally accommodated within the specified radius.
