This position requires the ability to perform evaluations, develop treatment plans, and implement therapy interventions tailored to each patient's needs.Responsibilities
Conduct patient evaluations and assessments.
Develop individualized treatment plans based on patient needs.
Implement therapy interventions and monitor patient progress.
Coordinate care with interdisciplinary team members.Required Experience / Certifications / Licensure
Must have experience with OASIS and Epic.
At least 1 year of experience in home health required.
