Opportunity:

Physical Therapist - Physical Therapy

Job Details:

• Monday through Friday schedule from 8:00 AM to 5:30 PM
• Guaranteed 37 hours per week with flexibility up to 40 hours per week
• Physical Therapists rotate one Saturday every 6-8 weeks with a compensatory day off during the week
• No holiday rotation required
• Occasional travel to other branches as needed
• Mileage reimbursed at government approved rate
• Evaluate, treat, and plan patient care in accordance with Physical Therapy standards of practice
• Serve adult and geriatric patient population with a variety of orthopedic, neurological, and functional mobility diagnoses
• Meet productivity expectations of an average of 4.5 visits per day
• Document to meet regulatory and reimbursement requirements using laptop for point of service documentation
• Utilize NetSmart EMR system and cell phone for GPS and communication
• Minimum of a Master's degree in Physical Therapy required
• Bachelor's degree required
• Home health experience required with training working with adult and geriatric populations
• Knowledge of Physical Therapy standards and practices required
• Required certifications: AHA BLS
• Wisconsin state licensure required
• NPI number required; candidates without one must begin the application process prior to start
• Must live outside a 50-mile radius of the primary location
