Job Summary: Physical Therapist specializing in Home Health care in Santa Rosa, California. This position involves providing outpatient therapy services to geriatric patients within a designated territory. The role requires flexibility in territory management and adherence to all clinical documentation and communication standards.
Job Details:
Schedule: Dayshift, 8-hour shifts, minimum of 40 hours per week starting from 08/31/2026
Required Qualifications: Certification in Physical Therapy, active license in hand, minimum of 1 year of recent relevant home health experience, proficiency in Homecare Homebase (HCHB), valid drivers license and personal transportation, ability to complete all charting by 9 a.m. the following morning, and scheduled MONDAY orientation.
Preferred Qualifications: Experience with OASIS documentation, additional home health certifications preferred.
Additional Information:
All charting must be completed by 9 a.m. the following morning.
On-call rotation may be required; discussion during interview recommended.
Telehealth or mobile documentation permitted with provided iPad.
Travelers expected to be flexible with territory up to a 60-mile radius from the designated hub or office.
RTO (Requested Time Off) must be submitted at application.
Former permanent employees must be separated for a minimum of 6 months before returning as travelers and must be HR approved.
Modules are billable and completed during orientation.
All travelers are required to have an active drivers license and personal transportation.
