Job Summary
Physical Therapist (Home Health) in a remote territory. This role involves providing skilled therapy services to geriatric patients through daily visits, ensuring timely documentation, and maintaining communication with the healthcare team. The position offers a flexible territory within up to a 60-mile radius from the hub.

Job Details

• Certifications: BLS certification required
• Education Requirements: License in hand, active drivers license, and personal transportation required
• Years of Experience: Minimum of 1 year of relevant experience, with at least 6 recent daily visits or a minimum of 30 points weekly for productivity
• Schedule: Day shift, 8 hours per day, minimum of 40 hours per week
• Additional Requirements: Timely documentation and communication are mandatory; RTO (Requested Time Off) must be submitted at the time of application; on-call rotation may be required, to be discussed during interview; no first-time travelers accepted; modules are billable and completed during orientation
• Preferred Qualifications: OASIS experience required for staff completing Start of Care in Home Health; proficiency in Homecare Homebase (HCHB) is required
• Responsibilities: Deliver targeted therapy services to geriatric patients, document progress, and collaborate with interdisciplinary team
• Submission Requirements: All candidates must have an active license, and submit RTO requests at application; RTO must be approved by HR. Former employees must be separated for at least 6 months before returning as a traveler.
