Key Responsibilities

• Assess patients' conditions and develop individualized plans of care to maximize mobility and independence
• Collaborate with an interdisciplinary team to ensure patients' needs are met
• Perform standard therapy measurement techniques including ROM, strength, and balance testing
• Utilize a variety of therapeutic modalities to maximize care provided to patients in a home setting
• Provide case management responsibilities as part of the home health care team

Required Qualifications

• Active Vermont Physical Therapist licensure
• Home Health experience required
• OASIS experience required
• Ability to provide one-on-one therapy care in a patient home setting

Preferred Experience / Skills

• Experience with McKesson/Netsmart EMR system strongly preferred
• Prior case management experience in a home health environment
