Description

American Traveler is seeking a Physical Therapist for a home health position requiring at least 1 year of home health PT experience and a valid driver's license.

Details

• Home health setting covering south Lake County
• Ortho case load with 4-6 visits per day
• Schedule is Monday through Friday, 8:30am-5pm
• 1 weekend rotation per month required for ortho case visits
• Coverage required for 1 major and 1 minor holiday
• No on-call requirement

Requirements

• Minimum 1 year of home health PT experience required
• Valid driver's license and auto insurance required
• License required if applicable
• SSN and DOB required for consideration
• 1 professional reference from entire work history required
• COVID vaccination card or approved declination required

Additional Information

• Weekend or holiday call-offs may result in rescheduling to another weekend or holiday per facility policy
• Mileage varies by week based on caseload
• Local candidates accepted at the same rate as travelers
• No radius rule in place
• Former permanent employees must have a 90-day break in service before returning as a traveler
