Job Summary:
Physical Therapist (PT) specialized in Home Health (HH) services is needed for a position located on the Leeward side of the island. The role involves planning and conducting segments of treatment programs aimed at restoring and improving patients' physical, social, and mental functions. The PT will serve as a co-leader within an interdisciplinary team and will perform comprehensive evaluations, establish individualized treatment plans, and document clinical activities accurately. This position requires active collaboration with patients and team members to support quality care delivery.

Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Bachelors degree, 2 years of physical therapy experience, Hawaii state physical therapist license, Epic electronic health record experience, travel experience, current CPR and BLS (AHA) certifications, valid driver's license, and current auto insurance
• Preferred Qualifications: Masters degree, 1 year of physical therapy home health experience
• Certifications: CPR, BLS (AHA)
• State Licenses: Hawaii
• Experience: Minimum 2 years of relevant physical therapy experience in home health setting
• Start Date: 09/08/2026
• Travel: Mileage reimbursement at 0.43 cents per mile, must have own transportation, and be prepared for license procurement procedures including in-person presence at the Hawaii Nursing Board
