Description

American Traveler is seeking a licensed Physical Therapist for a Home Health position in the Santa Rosa area, requiring a minimum of 1 year of home health experience and BLS certification.

Details

• Home health setting serving geriatric patients with a focus on home health and wound care cases
• Territory coverage will be discussed during the interview; clinician may be asked to travel to surrounding counties
• Minimum of 23–24 patient visits per week
• Day shift schedule, Monday–Friday, 8:00 AM–5:00 PM (5x8-hour shifts)
• No weekend or on-call requirements; on-call is discretionary only
• Charting done via Kinnser/WellSky; all charting must be completed on-site and cannot be taken home
• Mileage reimbursed at IRS standard rate from first to last patient visit (not to/from home)
• Free residential parking available

Requirements

• Minimum 1 year of home health experience and 2 years of overall clinical experience as a PT required
• Current, valid state PT license required; license required if applicable
• Current BLS certification required
• No hits or encumbrances on license; pending licenses are not accepted

Additional Information

• Contract length is 4–6 months; less experienced clinicians are required to complete a 6-month contract, which is non-negotiable
• Compliance documentation must be completed before the clinician can begin seeing patients; this process may take 2–3 weeks
• A facility-provided tablet is used for on-site charting
• Any color scrubs and closed-toe shoes required
• COVID vaccination required; religious declinations are accepted. Flu vaccination declination is accepted with documentation
• All time-off requests must be submitted at the time of initial submission; no additional RTO will be accepted after an offer is made
• Local candidates are accepted; candidates living within 50 miles of the facility qualify under the local rate
• First-time travelers are welcome to apply
• Hiring process includes a phone interview conducted by the hiring team prior to any offer being made
