Job Summary: Physical Therapist (PT) specializing in Home Health in an undefined location. This position involves providing physical therapy services to geriatric patients within a designated territory, ensuring timely documentation and communication. The role requires active licensure and proficiency with relevant clinical systems.
Job Details:
Certifications, Education, and Experience: Current BLS certification, at least 2 years of relevant experience including familiarity with OASIS for Start of Care processes, and active license in hand.
Schedule: Days shift, 8 hours per shift, with a minimum of 40 hours per week.
Additional Requirements: Flexibility with territory expected; transportation is mandatory; timely documentation is required; Monday orientation.
Responsibilities: Deliver physical therapy services to geriatric patients, perform timely assessments, documentation, and communication, and adhere to clinical and operational protocols.
Submission requirements: Active drivers license and personal transportation; RTO must be submitted at application.
Preferred Qualifications: Experience with Homecare Homebase (HCHB), OASIS, and outpatient rehabilitation in various settings such as SNF, outpatient clinics, and rehabilitation hospitals.
Float: Float rotation may be required.
