Job Summary
Physical Therapist Home Health (PT HH) Flexible location within a 60-mile radius of the hub. This role involves providing outpatient rehabilitative services to geriatric patients in a home health setting. The position emphasizes timely documentation, effective communication, and patient-centered care for a diverse patient population.

Job Details

• Required Qualifications: Valid license in hand, minimum of 2 years of relevant experience including OASIS experience for staff completing Start of Care (SOC) in home health, proficiency in Homecare Homebase (HCHB), and BLS certification.
• Schedule: Dayshift, 8-hour shifts, minimum of 40 hours per week.
• Responsibilities: Deliver outpatient rehabilitation therapies, perform charting and documentation, and coordinate with interdisciplinary teams.
• Preferred Qualifications: Experience with geriatric patient care and home health settings.
• Additional Information:
  • Home health experience and proficiency in OASIS are required for staff completing SOC (Start of Care).
  • Travelers must be flexible with territory and demonstrate timely communication and documentation.
  • Active driver's license and personal transportation are mandatory.
  • RTO (Requested Time Off) must be submitted at the time of application.
  • On-call rotation may be required and will be discussed during the interview.
  • The position requires an orientation on Monday.
  • Modules are billable and completed during orientation.
