Job Summary Physical Therapist (PT) Home Health The organization is seeking a licensed Physical Therapist to provide home health services to geriatric patients in the Mossyrock and Morton area. This role involves delivering high-quality patient care, completing OASIS assessments, and maintaining timely documentation within the specified territory.
Job Details
Position: Physical Therapist (PT) Home Health
Certifications, Education, and Experience: BLS certification; active license required; minimum of 1 year of relevant experience; experience with OASIS (Start of Care) assessments in home health settings preferred.
Schedule: Days, 8-hour shifts, with a minimum of 40 hours per week.
Additional Requirements: Must be flexible with the territory; timely communication and documentation are mandatory; on-call rotation may be requireddiscuss during interview.
Other Details: Home health experience with proficiency in HCHB (Homecare Homebase) required; no first-time travelers accepted; RTO must be submitted at application; Monday orientation; personal transportation and active driver's license required; modules are billable and completed during orientation; on-call rotation every sixth weekend.
