Key Responsibilities

• Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
• Serve a diverse patient population ranging in age from birth to over 100 years
• Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate appropriate services
• Maintain thorough and timely electronic documentation in accordance with agency policies, procedures, and standards of practice
• Participate in interdisciplinary communications to support coordinated patient care
• Travel to patient cases as assigned across the service area, including weekend rotations approximately once every six weeks

Required Qualifications

• Active Physical Therapist license in the state of Massachusetts required
• Minimum of 1 year of home care experience required
• Ability to travel to patient cases as assigned
• Availability for weekend rotations (approximately once every 6 weeks)

Preferred Experience / Skills

• Experience working with patients across a wide age range, including pediatric and geriatric populations
• Familiarity with electronic medical record (EMR) documentation systems
• Strong interpersonal and communication skills for effective interdisciplinary collaboration
