Job Summary Physical Therapist Home Health (PT HH) This position is based in an identified geographic area within a 60-mile radius of the hub. The role involves providing outpatient therapy services to geriatric patients in a home health setting, focusing on improving patient mobility and function. The position supports a dynamic team committed to delivering patient-centered care.
Job Details
Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Valid state license in hand, BLS certification, a minimum of 1 year of relevant experience, including OASIS experience for staff completing Start of Care (SOC), proficiency in Homecare Homebase (HCHB), and flexibility with territory.
Schedule: Monday through Friday, 8-hour shifts, minimum of 40 hours per week.
Additional Information:
Home health experience is required.
RTO (Requested Time Off) must be submitted at the time of application.
On-call rotation may be required, to be discussed during interview.
Timely communication and documentation are mandatory.
All travelers must have an active driver's license and personal transportation.
Orientation begins on Monday.
Modules are billable and completed during orientation.
Previous travel experience is not required; first-time travelers are accepted.
No dress code restrictions besides scrubs in any color, no jeans.
