Responsibilities

• Provide physical therapy services to patients with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes.
• Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services to meet those needs.
• Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies and procedures.

Requirements

• Minimum of 2 years of experience in physical therapy.
• Physical Therapist Professional license in Massachusetts.
• 1 year of Certified Home Care experience.
• 1 year of Case Management experience.
• Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), and Neonatal Resuscitation Program (NRP) certifications required.
• Ability to travel to cases as assigned.
