Job Details:

• Required Qualifications: Valid Physical Therapist Professional license in Massachusetts; 1 year of home care experience; 1 year of case management experience; experience in hospice, private care, or case management outside of a certified home health agency does not meet the qualification requirement.

• Float: Float: Float rotation may be required.
• Additional Information: The position covers a variety of neighborhoods including Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End. The start date is contingent upon credentialing clearance; if not cleared by the posted start date, a scheduled start date will be necessary.
