Description

American Traveler is seeking a Home Health Physical Therapist for a float position requiring 1 year of Certified Home Health Care experience and 1 year of Case Management experience.

Details

• Home health setting covering the North Region, including Chelsea, East Boston, Revere, and Winthrop
• 13-week assignment with a schedule of 8:00 AM to 4:30 PM
• Weekend rotation required once every 6 weeks
• Ability to travel to patient cases as assigned within the coverage area
• Uses Epic for charting and documentation
• Uses UKG/Kronos for time keeping

Requirements

• Minimum 1 year of Certified Home Health Care experience specifically within a certified home health agency
• Minimum 1 year of Case Management experience
• Hospice, private care, or standalone Case Manager experience does not meet the qualifying experience requirement
• License required if applicable
• Active MA license required at time of consideration; pending or conditional licenses not accepted
• AHA certifications only
• Flu vaccine required with no religious or medical exemptions allowed

Additional Information

• Responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and families
• Performs admission processes and case management duties in accordance with organizational policies and standards of practice
• Requires thorough and timely electronic documentation and interdisciplinary communication
• Start date is contingent on credentialing clearance; delays may result in a rescheduled start date
• Local candidates accepted if permanent residence is within 60 miles of Boston, MA, at a reduced rate
