Job Details:

Experience in hospice, private care, or solely as a case manager without home health agency experience does not meet the qualifications. Ability to travel to cases as assigned. Weekend rotations are once every 6 weeks. No experience minimum is listed apart from the specified years.
• Additional Information: Responsibilities include performing admission processes and case management duties, documenting electronically and communicating effectively with interdisciplinary teams. The start date is contingent upon credentialing clearance; if not cleared by the posted start date, a rescheduled start will be necessary.
