Description

American Traveler is seeking a Home Health Physical Therapist with a valid CA PT license and Epic/OASIS experience to serve the South Marin County area.

Details

• Home Health setting covering South Marin County territory, including Sausalito, Tiburon, Belvedere, and Mill Valley
• 13-week assignment with a Day shift schedule (5x8 hours, 8:00 AM - 5:00 PM)
• Caseload of approximately 5 visits/points per day
• Utilizes Epic EMR with OASIS documentation required

Requirements

• Current CA PT License required (license required if applicable)
• BLS certification required
• Valid driver's license and auto insurance required
• Minimum 1 year of experience required
• Prior travel experience required
• Epic EMR experience and OASIS documentation experience required
• Experience with orthopedic, neurologic, and cardiopulmonary patient populations preferred
• Skilled in gait training, therapeutic exercise, wound/skin assessment, and home exercise program development
• Strong critical thinking, independent judgment, and time management skills required
• Effective verbal and written communication and interpersonal skills required

Additional Information

• Responsibilities include patient assessment, evaluation, treatment planning, and discharge planning within a home health setting
• Collaborates with interdisciplinary team members and key stakeholders to develop patient care solutions
• Provides patient and family education on home exercise programs, safety, and injury prevention
• Maintains awareness of professional practice standards and regulatory requirements
• Accepts local candidates at the local rate
• 50-mile radius rule applies for travel rate eligibility
• Returning employees must have a 1-year separation before returning as a traveler
• Only 1 holiday time-off request approved per contract
• Maximum RTO of 7 days per contract
