Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring 1 year of certified home care and case management experience.

Details

• Home health setting covering Chelsea, East Boston, Revere, and Winthrop
• 13-week assignment with an 08:00-16:30 shift schedule
• Weekend rotation occurs once every 6 weeks
• Requires travel to patient homes as assigned
• Uses Epic for charting

Requirements

• Minimum 1 year of Certified Home Health Care experience required
• Minimum 1 year of Case Management experience required
• Experience must be specifically within a certified home health agency; hospice, private care, or standalone case management roles do not qualify
• License required if applicable
• Flu vaccination required with no religious or medical exemptions accepted
• Ability to travel between patient care locations as needed

• Responsibilities include assessing patient conditions, implementing care plans, and providing education and support to patients and families
• Performs admission processes and case management duties per organizational policy and standards
