We’re hiring a Home Health Physical Therapist for an exciting travel assignment in Northern California!

Position Details:
• Mileage reimbursement in addition to pay package
• Home Health setting
• 40 hours guaranteed

Requirements:
• Active PT License required
• DPT required if licensed after January 2016
• BLS/CPR certification required
• Valid Driver’s License required
• Must be 21 years or older
• Home Health experience preferred (1 year ideal, willing to train)

Responsibilities:
• Perform patient evaluations and individualized treatment planning
• Provide therapy interventions and discharge planning
• Complete clinical documentation accurately and timely
• Supervise assistants as assigned
