Schedule: Monday–Friday | Day Shift (1st 8h)

Role Overview

We’re seeking an experienced Physical Therapist – Home Health to deliver personalized, high-quality care across a diverse and scenic Northern California service area. You’ll support patients recovering and regaining independence in their own homes—making every visit meaningful and impactful.

WHAT YOU’LL DO
• Perform comprehensive patient evaluations and develop individualized treatment plans
• Deliver evidence-based physical therapy interventions in home settings
• Establish and manage care and discharge plans
• Maintain timely and accurate clinical documentation
• Supervise therapy assistants as needed
• Collaborate in an interdisciplinary care model to optimize patient outcomes

Requirements

Doctor of Physical Therapy (DPT) – required for licenses issued after Jan 2016
• Active Physical Therapy license (CA and/or applicable states)
• Current CPR/BLS certification
• Valid driver’s license
• Home health experience preferred (1+ year ideal, but willing to train motivated candidates)
