Travel Physical Therapist – Home Health

We’re seeking an experienced Travel Physical Therapist for a Home Health assignment in Sonora, California. This role is ideal for PTs who are comfortable working independently and managing a home health caseload.
Location: Sonora, CA
Setting: Home Health
Duration: 13 Weeks (extension possible)
Schedule: Full-time (based on patient census)
Minimum 2 years of Physical Therapy experience – Preferred
Home Health experience – Required
HCHB (Homecare Homebase) experience – Required
OASIS documentation experience – Required
Active California PT License – Required
BLS/CPR Certification – Required
Valid Driver’s License – Required
Provide in-home physical therapy evaluations and treatments
Complete OASIS assessments and documentation accurately
Develop and implement individualized care plans
Educate patients and caregivers on therapy plans and safety
Collaborate with interdisciplinary home health teams
Maintain compliance with state and federal home health regulations
