Schedule: 8-hour shifts, 5 days per week (8x5s)
Duration: 13 weeks
Home Health experience – Required
Active California Physical Therapist License – Required
BLS/CPR Certification – Required
Valid Driver’s License – Required
Must be 21 years of age or older
Master’s Degree in Physical Therapy – Preferred
Conduct comprehensive patient evaluations to assess functional limitations and rehabilitation needs
Develop and implement individualized treatment plans tailored to each patient’s goals
Address clinical concerns identified during assessments and adjust care plans as needed
Maintain accurate, timely documentation in accordance with regulatory and agency standards
Ensure compliance with state, federal, and organizational guidelines
Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes
