Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR certification; active applicable state license; 0 years experience required; 09/21/2026 start date; end date 12/21/2026
Preferred Qualifications: Successful prior assignment with the agency; previous experience with the organization; 6-month duration desired
Additional Information: Completes comprehensive evaluations, develops individualized treatment plans, and ensures documentation meets all regulatory and organizational standards. Participates in student and professional development programs. Maintains licensure, follows performance improvement initiatives, and adheres to all professional and safety standards.
