Job Summary: Physical Therapist Outpatient (PT OP) in Norwalk, Connecticut. This outpatient position involves treating ortho and potential neuro patients within a physical medicine and rehabilitation setting. The role includes performing physical therapy evaluations, developing treatment plans, and providing patient and family education.
Job Details:
Location: 34 Maple Street, Norwalk, CT 06856
Schedule: Days, 8 hours shifts, minimum of 40 hours per week
Required Qualifications: Connecticut Physical Therapist (PT) license, Basic Life Support (BLS) certification, at least 1 year of experience working with patients with neurological conditions
Preferred Qualifications: Vestibular therapy experience; no on-call or floating responsibilities
Responsibilities:
Treat outpatient physical therapy patients with orthopedic and neurological conditions
Conduct physical therapy evaluations and implement treatment plans
Provide patient and family education and training
Complete documentation timely per department guidelines
Initiate discharge planning and perform other duties as assigned
