Job Summary: Physical Therapist (PT) specializing in Home Health (HH) services is sought for a daytime assignment in a healthcare organization. The position involves providing specialized pelvic floor rehabilitation services to a diverse adult patient population. This role requires a highly qualified PT with postgraduate training in pelvic health and at least two years of relevant clinical experience. The selected candidate must be prepared to work a consistent day shift with a schedule of 8 hours per day and a minimum of 40 hours per week.

Required Qualifications: Degree in Physical Therapy, PT license, state licensure, 2 years of physical therapy experience, completion of postgraduate instruction in pelvic health from Herman and Wallace and/or the American Physical Therapy Association, Basic Life Support (BLS) certification, First Aid certification preferred, primary source online verification required
Responsibilities: As a member of an interdisciplinary team, provide high-quality physical therapy services including comprehensive evaluation, report development, and treatment planning for adult patients over age 18. Perform detailed clinical assessments based on individual needs, develop comprehensive age-appropriate plans of care following physician referrals, accurately document treatments, and respond effectively to changing work assignments. Caseload consists entirely of pelvic floor rehabilitation across all genders.
Additional Information: Must reside within 50 miles of the service area and be available to work one holiday if needed. Experience in pelvic health rehabilitation with coursework from Herman and Wallace and/or APTA is required.
