We are seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team in a Skilled Nursing Facility (SNF). The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide high-quality therapy services to help patients restore mobility, improve strength and balance, reduce pain, and maximize functional independence.

Responsibilities- Evaluate patients and develop individualized physical therapy treatment plans
- Provide therapeutic exercises, gait training, balance training, transfer training, manual therapy, and functional mobility interventions
- Assess patient progress and modify treatment plans to achieve optimal rehabilitation outcomes
- Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safety strategies
- Maintain accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory standards
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary healthcare team
- Participate in discharge planning and recommend continued therapy services when appropriate
- Ensure compliance with all facility, state, and federal regulations
- Deliver compassionate, patient-centered care focused on improving function, mobility, and quality of life
Requirements- Required: Graduate of an accredited Physical Therapy program
- Required: Active Physical Therapist (PT) license in the state of practice or ability to obtain
- Preferred: Previous experience in a Skilled Nursing Facility (SNF) or post-acute care setting
- Strong communication, documentation, organizational, and interpersonal skills
- Ability to work independently and collaboratively within a multidisciplinary team
